You’re right it is a double entender but what it’s talking about is him losing his patience and it being a test to him as a child and as a young adult to watch his mother fade away because of her religion and her faith in something that did not help her. It’s also a case of telling the story from her point of view where she is losing patience but never actually losing faith because she continually believes in God even though God did nothing for her and never can or could.
Literally the verses are changing back and forth between points of view of him and his mother sometimes the lines themselves change points of view but it’s literally a conversation psychologically between two people one who can’t speak and one who can.
